Role Overview
We are seeking an experienced and motivated Clinical Lead to provide clinical oversight, leadership, and governance across our complex care packages. The successful candidate will support the safe delivery of delegated healthcare tasks, oversee clinical risk management, guide care staff, and ensure care plans, competencies, documentation, and clinical practices remain compliant with CQC standards, company policies, and best practice guidance.

Responsibilities

The Clinical Lead will:

  • Provide clinical leadership and guidance to care teams supporting individuals with complex health needs.
  • Oversee clinical care plans, risk assessments, health action plans, escalation plans, and emergency protocols.
  • Support staff competency assessments for delegated healthcare tasks, including PEG care, suctioning, epilepsy care, rescue medication, diabetes care, catheter care, respiratory support, and other clinical interventions as required.
  • Review clinical documentation, daily notes, MAR/eMAR records, monitoring charts, incident reports, and clinical observations.
  • Identify clinical risks, changes in health needs, safeguarding concerns, medication issues, and documentation gaps.
  • Support care reviews, MDT meetings, hospital discharge planning, and commissioner reviews.
  • Provide coaching, supervision, and clinical guidance to care staff and senior staff.
  • Ensure timely escalation to GPs, community nurses, specialist teams, emergency services, commissioners, and family representatives where appropriate.
  • Contribute to audits, quality assurance, governance meetings, lessons learned, and service improvement.
  • Promote safe, person-centred care in line with CQC Fundamental Standards, NICE guidance, safeguarding principles, MCA, GDPR, infection prevention and control, and medication safety requirements.

Requirements & Qualifications

Essential Requirements

  • Registered Nurse with active NMC registration.
  • Experience in complex care, community care, domiciliary care, supported living, or adult social care.
  • Strong knowledge of clinical risk assessment, care planning, medication safety, safeguarding, MCA, infection control, and CQC requirements.
  • Experience supporting or overseeing delegated healthcare tasks.
  • Ability to lead, guide, and support care staff in a professional and practical manner.
  • Excellent written communication and documentation skills.
  • Strong organisational skills and ability to manage competing priorities.
  • Commitment to dignity, respect, confidentiality, professional boundaries, and person-centred care.

Desirable Experience

  • Experience with PEG feeding, epilepsy care, suctioning, respiratory support, diabetes care, catheter care, pressure care, and complex moving and handling.
  • Experience completing clinical audits, competency assessments, governance reports, and incident reviews.
  • Previous leadership experience within a CQC-regulated service.
  • Experience using electronic care planning or eMAR systems.

Skills

The successful Clinical Lead will need to demonstrate strong clinical knowledge, leadership ability, and a clear commitment to safe, person-centred care. They should be confident in supporting individuals with complex health needs and able to provide guidance, supervision, and clinical oversight to care staff.

Essential skills include:

  • Strong clinical judgement and the ability to identify changes in health, deterioration, risk, or safeguarding concerns.
  • Experience supporting or overseeing complex care needs such as PEG feeding, epilepsy care, suctioning, respiratory support, diabetes care, catheter care, pressure care, medication support, and complex moving and handling.
  • Ability to review and develop care plans, risk assessments, clinical protocols, escalation plans, and health monitoring records.
  • Good understanding of delegated healthcare tasks, staff competency assessments, and safe clinical practice.
  • Knowledge of medication safety, infection prevention and control, safeguarding, Mental Capacity Act, consent, confidentiality, and professional boundaries.
  • Ability to lead, supervise, coach, and support care staff in a calm and professional manner.
  • Excellent verbal and written communication skills, including clear documentation, handovers, reports, and professional correspondence.
  • Ability to liaise effectively with families, GPs, community nurses, social workers, commissioners, hospital teams, and other professionals.
  • Strong organisational skills, with the ability to prioritise workload, manage clinical risks, and follow up actions promptly.
  • Good audit and governance awareness, including the ability to review MAR/eMAR records, daily notes, monitoring charts, body maps, incident reports, and care documentation.
  • Confidence in identifying documentation gaps, practice concerns, training needs, and areas for service improvement.
  • A compassionate, respectful, reliable, and accountable approach, with a strong commitment to dignity, safeguarding, confidentiality, and high-quality care.
