We are looking for an experienced, organised and proactive Care Coordinator with previous complex-care experience to join our team and support the safe and effective coordination of our services across London.

The Role

The Care Coordinator will play a key role in the day-to-day management and coordination of complex care packages.

The successful candidate will work closely with service users, families, care staff, management, healthcare professionals and commissioners to ensure that care is delivered safely, consistently and in accordance with each person’s assessed needs, care plans and risk assessments.

As the role involves travelling between service users’ homes, completing visits, supporting staff and attending meetings where required, applicants must hold a full UK driving licence. A company vehicle can be provided for work-related travel.

This position would particularly suit an experienced Care Coordinator, Senior Care Worker or similar professional who has a good understanding of complex home-care packages and is ready to take responsibility for coordinating high-quality care.

Responsibilities

Key Responsibilities

The Care Coordinator will be responsible for:

  • Coordinating complex care packages and ensuring appropriate staffing is maintained.
  • Preparing and managing staff rotas to ensure continuity of care and an appropriate skill mix.
  • Matching care staff to service users according to their needs, preferences, competencies and assessed risks.
  • Ensuring shifts are covered and responding promptly to sickness, absence and other staffing changes.
  • Maintaining effective communication with service users, families, care staff and professionals.
  • Visiting service users in their homes as required to monitor service quality and identify any changes in need.
  • Supporting the development and review of person-centred care plans and risk assessments.
  • Ensuring staff understand and follow current care plans, risk assessments and clinical guidance.
  • Supporting the safe delivery of delegated healthcare tasks where these form part of an individual’s care package.
  • Monitoring staff training and competencies relevant to the needs of the people they support.
  • Supporting packages involving complex needs such as PEG/gastrostomy care, epilepsy, rescue medication, suctioning, mobility support and other delegated healthcare interventions, where applicable.
  • Monitoring care records, daily notes, medication records and other documentation for completeness, accuracy and quality.
  • Identifying and escalating concerns relating to health deterioration, medication, safeguarding, staffing, missed care or changes in a service user’s needs.
  • Supporting incident and accident reporting, investigation and follow-up actions.
  • Supporting safeguarding processes and escalating concerns appropriately.
  • Liaising with GPs, community nurses, social workers, commissioners, therapists and other multidisciplinary professionals where required.
  • Supporting hospital admissions, discharge planning and changes to care packages.
  • Conducting or supporting service-user reviews, staff spot checks and supervision where required.
  • Completing monitoring visits and quality-assurance checks across care packages.
  • Monitoring service quality and raising areas requiring improvement with the Registered Manager.
  • Supporting new care package mobilisation and ensuring appropriate documentation, staffing and competencies are in place before services commence.
  • Attending service-user meetings, multidisciplinary meetings and commissioner meetings where required.
  • Maintaining accurate, confidential and audit-ready records.
  • Supporting compliance with company policies, CQC requirements, safeguarding procedures, the Mental Capacity Act, medication procedures, infection prevention and control and data protection requirements.
  • Participating in management meetings and providing updates regarding staffing, incidents, risks and service delivery.
  • Supporting the Registered Manager and wider management team with the effective operation of the service.
  • Travelling between service-user locations and other work-related locations as required.

Requirements & Qualifications

Essential Requirements

Applicants must have:

  • Previous experience working as a Care Coordinator or in a similar care coordination role.
  • Previous experience supporting or coordinating complex care packages.
  • Experience within domiciliary care, home care, supported living or a similar regulated care environment.
  • Experience coordinating staff rotas and managing shift coverage.
  • Experience working with people with complex physical health, neurological or disability-related needs.
  • Good understanding of safeguarding adults.
  • Good understanding of person-centred care, dignity, choice and independence.
  • Ability to recognise health deterioration, care concerns and risks and escalate appropriately.
  • Understanding of care planning and risk assessment.
  • Good knowledge of medication safety and care documentation.
  • Strong organisational and time-management skills.
  • Excellent written and verbal communication skills.
  • Ability to communicate professionally with service users, families, healthcare professionals and commissioners.
  • Confidence managing challenging situations and competing priorities.
  • Good IT skills and ability to use electronic care-management and rostering systems.
  • Ability to maintain accurate and confidential records.
  • Ability to work effectively both independently and as part of a management team.
  • A professional, calm and solution-focused approach.
  • A full UK driving licence.
  • Ability and willingness to travel between service-user locations across London as required.

Desirable Requirements

It would be advantageous for applicants to have:

  • Level 3 Diploma in Adult Care or equivalent.
  • Level 4 or Level 5 qualification in Adult Care, Leadership or Management.
  • Experience within a CQC-regulated service.
  • Experience coordinating 24-hour or high-dependency complex care packages.
  • Experience supporting people requiring PEG feeding, suctioning, epilepsy management, rescue medication or other delegated healthcare interventions.
  • Experience conducting care reviews, risk assessments, spot checks or staff supervisions.
  • Experience working with multidisciplinary teams and local authority or NHS commissioners.
  • Knowledge of the Mental Capacity Act and best-interest decision-making.
  • Experience supporting service mobilisation or transitioning new complex care packages into the community.
